# Paced Breathing With Haptics: Why Feeling the Rhythm Beats Watching a Circle

That is a smaller claim than most apps make, and it is the honest one.

The argument for a tactile cue is about what a visual pacer costs you, not about a different physiological effect. To follow an expanding circle you have to keep your eyes open, keep a lit screen in your field of view, and give a slice of attention to tracking a shape. At 11pm, or in an office where you would rather not be seen watching a meditation animation, those are real costs. A vibration removes all three. Whether that produces a measurably better outcome has not been tested, and you should treat anyone claiming otherwise with suspicion.

## How to do it

1. Pick a pattern before you start. Deciding mid-session pulls you back out.
2. Sit upright or lie down. Hold the phone flat in your palm, or rest it on your chest just below the collarbone. The chest position puts the cue next to the movement you are changing.
3. Close your eyes. There is nothing to look at, which is the point.
4. Let the first cue arrive before you do anything. Do not anticipate it.
5. Inhale through your nose. Exhale through your nose or a slightly open mouth, whichever is quieter for you.
6. Do not chase the pattern. If a phase feels long, take a smaller breath rather than straining to fill the whole interval.
7. If you lose the thread, breathe normally for one cycle and rejoin at the next inhale cue.
8. When the rounds end, stay still for another twenty or thirty seconds with no pattern at all. Most of the settling shows up there, not during.

## What the evidence says

The evidence sits on the breathing rate, not on the cue. Zaccaro and colleagues (2018), in a systematic review in Frontiers in Human Neuroscience, screened 2,461 abstracts and included 15 studies of slow breathing at under roughly ten breaths per minute. Across them, slow breathing was associated with increased heart rate variability and respiratory sinus arrhythmia, increased alpha and decreased theta activity in the EEG, and in several studies with reduced anxiety and self-reported arousal. Fifteen studies with small samples is a modest base, and the review says so.

Evidence for haptic pacing specifically is thin. Vermeylen and colleagues (2022) ran a mixed-method pilot in Frontiers in Digital Health with 39 adults using a handheld tactile breathing device; subjective sleep quality improved by about three points on the Pittsburgh Sleep Quality Index. There was no control group, so expectancy and simple attention to sleep cannot be separated from the device. Honinx and colleagues (2025), in the International Journal of Clinical and Health Psychology, used a cross-over design with 36 stressed adults and two tactile devices. Respiratory rate fell with both, one device showed EEG changes, and most participants preferred the handheld one. The authors note there was no control or placebo condition.

So: the slow-breathing effect is reasonably supported and modest. Tactile delivery of it has been studied in small, uncontrolled samples, and no trial has compared a haptic pacer against a visual one for outcomes. The technique is identical; only the cue changes, and the cue is a usability choice rather than a proven advantage.

## When it helps and when it doesn't

It helps when the obstacle is counting. People who lose track, or who count faster as they get more agitated, get a steadier rate from an external cue. It helps in bed, where a lit screen works against you. It helps in public, because a phone in your hand with your eyes closed looks like nothing in particular.

It does not help if you fight the rhythm. If a pattern leaves you air-hungry, the pattern is wrong for you today, not something to push through. Drop the holds and use coherent breathing instead. And it is not a treatment: paced breathing is not a substitute for care for panic disorder, an anxiety disorder, or chronic insomnia.

## Safety

If you are pregnant, have a cardiovascular or respiratory condition, or have epilepsy, talk to a doctor before practising breath holds or paced breathing. Stop if you feel dizzy or notice tingling in your hands or face, and breathe normally until it passes. Never practise while driving or in water. If you are already in panic, do not force a big inhale, which usually makes it worse. Start with a slow exhale and let the inhale look after itself.
