Coherent Breathing (Resonant Breathing): 5 Seconds In, 5 Seconds Out

Coherent breathing means five seconds in and five seconds out through the nose, no holds, no pause between them. That works out to six breaths a minute. The pace matters more than the effort: at around six breaths a minute, heart rate and breathing fall into step, the swing in heart rate with each breath grows large, and heart rate variability reaches its maximum. Researchers call that the resonance frequency, which is why the technique also goes by resonant breathing.
How to do it
- Sit upright or lie down. Let your shoulders and jaw go slack.
- Breathe in through your nose for five seconds. Aim for a light, unhurried breath. This is not a deep breath.
- Breathe out through your nose for five seconds, at the same easy volume.
- Do not hold at either end. The turn from in to out should be smooth and continuous.
- Six rounds is one minute. Continue up to thirty rounds, about five minutes, if you have the time.

The most common error is breathing too deeply. Volume is not the goal, timing is. If you feel short of air, you are pulling in more than you need; make each breath shallower while keeping the five-second count. If five seconds feels wrong, four or six will still land inside the useful range.
What the evidence says
Lehrer and Gevirtz set out the mechanism in their 2014 paper in Frontiers in Psychology. Breathing at resonance frequency, usually between about 4.5 and 6.5 breaths a minute in adults, drives the baroreflex, the loop that keeps blood pressure stable, into a large oscillation. Repeated practice appears to strengthen that reflex, which is the leading explanation for why heart rate variability biofeedback helps across a range of conditions. They are careful about the claim: the mechanism is well argued, the clinical effect sizes vary by condition, and the field is small.
Steffen and colleagues, in Frontiers in Public Health in 2017, tested this directly. Participants who breathed at their resonance frequency reported better mood after the exercise than comparison groups, and showed lower systolic blood pressure during a stress task and the recovery period afterwards. It is a small laboratory study with short-term outcomes, so treat it as support for the direction, not as proof of lasting blood pressure control.
Zaccaro and colleagues' 2018 systematic review puts coherent breathing in context. Across the slow-breathing literature, breathing under ten breaths a minute is consistently associated with higher heart rate variability, increased vagal activity, and reports of reduced arousal and improved comfort. The evidence is more consistent for physiological measures than for psychological ones, and most studies are small.
When it helps and when it doesn't
Coherent breathing suits sustained practice better than emergency use. Ten minutes daily is the shape most of the research uses, and it is a reasonable way to lower a chronically high baseline. It is also good before sleep, during recovery from exercise, and any time you want to settle without the sedating pull of a very long exhale.
For a sudden spike, it is slower to bite than an exhale-weighted pattern, since it takes several rounds to have an effect. If your arousal is already high, a physiological sigh first and coherent breathing after works better than starting here. It is not a treatment for hypertension, an anxiety disorder, or heart disease, and it does not replace medication or therapy.
Safety
If you are pregnant, or have a cardiovascular or respiratory condition, or epilepsy, talk to a doctor before taking up a breathing practice. Stop if you feel dizzy, lightheaded, or tingling in your hands or face, and return to normal breathing. Never practice while driving or in water.
